Copy the register structure

Use one entry per material risk and retain prior decisions when an entry changes. Keep an unresolved issue open until the response has been supported by evidence, not merely assigned to someone.

  • Risk reference and scenario: what could happen and under which condition.
  • Evidence and uncertainty: source, date, observed facts and remaining assumptions.
  • Impact: effect on cost, schedule, liquidity, permitted use or operations.
  • Current response: investigation, mitigation, transfer where appropriate, or a decision to avoid exposure.
  • Responsible person, next action and deadline: who will produce which evidence by when.
  • Decision trigger and residual exposure: when to escalate, pause or revise, and what remains after the response.

A completed illustrative entry

Risk: a funding delay could leave invoices due before the next loan draw. Evidence: a hypothetical payment schedule with timing still unconfirmed by the lender. Impact: a temporary cash shortfall and possible work interruption. Response: reconcile payment dates, draw conditions and available liquidity. Owner: the person coordinating finance. Next action: obtain and review the relevant funding conditions before the next spending commitment. Residual exposure: approval and processing delays can remain even after a schedule is prepared.

This is a teaching example, not a statement about an actual loan.

Review decisions, not just colors

Discuss changes in exposure and overdue actions at each relevant project review. A green label without evidence is not a resolution. Connect related risks: a delay can increase carry and equity needs while moving the opening into a different demand period.

Use qualified specialists for conclusions within their fields. The register coordinates business decisions; it does not eliminate risk or replace contractual protections.
